WHAT TO BRING TO A TAX APPOINTMENT:
· Valid photo ID for you and a spouse if filing jointly.
· If filing a joint return both parties must sign the return, and should be present at the meeting.
· Social Security cards or Tax Identification Numbers (ITIN) for everyone on the return
· Anything showing income from 2011, including a W-2, 1099, Interest Statement.
· 1099-HC from your health insurance company (Medicare recipients will not receive a 1099-HC, nor will MassHealth recipients with income below 150% FPL) (Health insurance card is fine if they didn’t receive a 1099-HC)
· A blank check if you would like any refund to be directly deposited in a bank account
· A copy of last year’s tax return (if possible)
You May Also Need to Bring:
· If you paid RENT for your place of residence in Massachusetts, please tally your rent paid in 2011 and bring the total number to your appointment.
· If you pay for childcare or other dependent care, please bring the provider’s full name, address, and amount paid. If the provider is an individual, please bring his or her Social Security number. If the provider is an organization, bring the employer identification number (EIN), also known as the tax ID.” Please note that care includes daycare, summer day camp, after school programs, or a babysitter paid with a W-2 (not under the table).
· If you or your spouse paid alimony in 2011, please bring a record of the amount paid or received.
· 1098-T or 1098-E if you, your spouse, or a dependent paid tuition or interest on a student loan in 2011.
Tax appointments are scheduled for an hour and a half. If you do not come during your scheduled time or fail to bring all the necessary documents you will likely have to wait 2-3 weeks to get another appointment. You will receive a reminder call the day before your appointment.
